About Us

Today, the team at CVNA provides skilled nursing, rehabilitative therapies, social work as well as volunteer and support services through five distinct programs: Home Care, Hospice Care, Palliative Care, Life Care-Private Duty and Community Care. CVNA is committed to bringing services to people of all ages, regardless of their ability to pay, so that families can stay together at home, even when facing the challenges of aging, surgical recovery, chronic or life-threatening illnesses or end of life care.

Through our core programs, CVNA provides specialized care including Wound, Ostomy and Incontinence Care, Diabetes Management and Education, Certified Intravenous Therapy, Chronic Care Management, a Certified Behavioral Health Program, a Balance Therapy and Fall Prevention Program, and a Maternal Wellness Program. Additionally, CVNA is committed to investing in advanced technology, and offers a sophisticated telehealth program for high risk patients.
